Montereau is currently seeking a Quality Care Coordinator to work days.\u00a0 This position will assist the Director of Nursing and Assistant Director of Nursing in supporting Health Center nursing operations to ensure the delivery of high quality and proficient care to residents, in accordance with federal and state regulations. Partner and collaborate with the Nursing Leadership Team to develop and assure sustained excellence in Montereau's Quality Assurance and Performance Improvement (QAPI) initiatives and activities in order to achieve a decrease in hospital admissions\/readmissions. Essential Functions: Assist the Director of Nursing and Assistant Director of Nursing in leading and managing Charge Nurses, CNAs, and CMAs to assure the desired level of resident service, care, and satisfaction. Participate in monitoring and evaluating employee performance and provide coaching, training, and corrective actions, as required. Assure equitable administration of policies and compliance with regulatory and legal requirements. Develop and provide appropriate recommendations for a range of employee and organizational matters, including staffing, promotion, and termination actions. Oversee, monitor, and administer a wide range of key nursing clinical programs to include: fall and injury prevention, management of infections, hydration, skin and wound care, medication to include antipsychotics and documentation of associated behaviors and side effects and oxygen administration. Ensure accurate and complete documentation of skilled and long-term care notes, perform root cause analysis to identify or develop appropriate plans of action to correct identified problems and deficiencies, implement identified plan of action, and monitor for appropriate outcomes. Develop and implement corrective action plans with nursing staff by providing direct feedback, recommending and\/or delivering disciplinary action, and training\/re-educating when critical issues and deficiencies are detected during audits or medical record reviews. Assure documentation of remediation and report these actions to the Director of Nursing. Monitor all admission and discharge documentation for accuracy and completeness and routinely assist Charge Nurses with admissions and discharges. Monitor and assess regulatory documentation for accuracy and completeness for the following: quarterly elopement assessments, temperature and glucometer control logs, coumadin\/international ratio (INR) tracking, labs, hospice documentation, and CNA documentation in point of care (POC) system, specifically activities of daily living (ADL) accuracy. Partner with Health Services Interdisciplinary Team (IDT) in the evaluation of resident care operations and affect changes as necessary to improve programs and assure compliance with regulatory requirements. Participate in the Risk Assessment Committee by developing written and oral reports and recommendations for the Director of Nursing concerning the operation of the QAPI Program and collaborate with the Committee to determine appropriate preventative measures. Assist in the investigation of allegations of resident abuse, neglect, misappropriation of resident property, and any other state reportable incident. Other Functions : Participate in the Health Centers Manager on Duty (MOD) rotation and the Health Centers' Nurse Manager On-Call rotation to support Charge Nurses with critical resident care decisions and assure adequate staffing and scheduling. Per rotating schedule, serve on Montereau's monthly Safety Committee by monitoring\/promoting safe practices, reporting unsafe acts, and offering improvement suggestions. Participate in staff meetings for licensed and non-licensed staff members. Assist the Staff Development Coordinator in providing a high quality orientation to new hire nursing staff. Work care base shifts, as needed, to assure proper resident care is provided. Perform other related functions as required or as assigned. Position Requirements Experience: A minimum of 5 years of experience within a long-term care (LTC), skilled nursing, or hospital environment is preferred Previous experience working with federal, state and local nursing services regulations and practical experience analyzing and interpreting statistical reports while working in an interdisciplinary team environment is required Previous experience within a Continuing Care Retirement Community (CCRC) is preferred Previous leadership experience in a health care setting is preferred Education\/Certifications\/Licenses: Current State of Oklahoma Registered Nurse license or Licensed Practical Nurse license is required \u00a0 Job Specific Knowledge, Skills, and Abilities: Understanding of state and federal regulations relating to LTC Strong resident care documentation skills Working knowledge of Microsoft Office programs and ability to learn new software systems (i.e. AOD) Working knowledge of Minimum Data Sets (MDS) is preferred Strong communication skills to effectively and empathetically address critical health and death issues with a diverse resident population and family members Solid problem solving skills and decision making skills Strong interpersonal skills and understanding of quality customer management and service Ability to effectively interact with a diverse leadership team, staff, residents, and third party associates, including vendors, providers, and visitors Ability to effectively manage sensitive and confidential communications and information, while maintaining a high level of poise, tact and diplomacy Physical Demands: In order to accomplish the essential functions of this position, the following are required: An extensive amount of walking, standing, and sitting, with a moderate amount of bending at the waist, kneeling, twisting\/turning, reaching above the head and shoulders, and eye-hand coordination Ability to routinely lift 25 pounds and push wheelchairs with loads of up to 300 pounds Frequent use of ordinary communication, including normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision, and depth perception Ability to use a computer or other office equipment for an extensive period of time Ability to occasionally travel by car and\/or commercial airline Working Conditions: This position is performed primarily indoors in both a private office environment and in a skilled nursing resident environment with occasional exposure to high noise levels (alarms). Mental and Emotional Requirements: This position requires: Understanding and carrying out oral and written instructions and reviewing\/preparing detailed reports, schedules and medical records, using measuring devices and simple calculations Planning own work activities and the activities of others, and directing and training others in their work assignments Working independently or as a member of a team, and the ability to work with limited supervision Routinely working on multiple tasks at the same time, under moderate amounts of pressure for results, with frequently changing priorities and requirements for extreme accuracy Safety\/Security Requirements: Must successfully pass a pre-employment drug test Must successfully pass a criminal background screening Must successfully pass a tuberculosis (TB) screening test
